They are designed to look like legitimate pharmaceuticals but may contain fentanyl, other novel synthetic opioids, and/or other drugs [11]. First reports about the counterfeit pills containing fentanyl emerged in the initial phases of the NPF epidemic in 2014–2015 [12]. Instead, powder-type NPF, often sold as heroin or mixed with heroin, became increasingly available, gradually saturating illicit drug markets and in some regions nearly replacing heroin [2, 12,13,14]. Since 2020, many states across the USA have registered notable shifts in the illicit market dynamics as NPF is increasingly available in counterfeit pills, not just in powder form and/or as heroin [8, 9, 11].

Counterfeit pills containing fentanyl vary in risk but can be deadly if they contain a dosage high enough to be fatal. The pills are stamped with an “M” logo on one end and the number 30 imprinted on the other. Increases in counterfeit pill presence have been especially dramatic in Arizona. Located on important drug trafficking routes, Arizona is considered to be the “ground zero” of the evolving NPF epidemic [15]. According to the Arizona Criminal Justice Commission, between 2019 and 2020 retail-level seizures of counterfeit pharmaceutical pills that contain fentanyl increased 764% in Arizona [7].

This powerful opioid binds to the brain’s opioid receptors, producing intense feelings of euphoria and relaxation. The rapid onset and intensity of these effects contribute to the drug’s addictive properties. They are immediate release pills used for post-operative or other moderate pain in a four to six-hour interval. They should be taken at the same time each day and exactly as prescribed by a doctor. Fake M30 oxycodone is reportedly being sold with lethal amounts of fentanyl, a synthetic opioid that is responsible for over 70% of opioid deaths annually. Fentanyl is incredibly potent and overdose can set in rapidly, with respiratory depression and unconsciousness often leading to death.
